In addition to touring our adult family home and meeting our staff, we welcome the opportunity to provide an assessment. With direct input from you, your loved one, and your physicians, a collaborative custom care plan will be developed to ensure a shared vision of needs and expectations.
Care plans will be regularly updated and available for family and physician review to confirm progress. Whether we are creating new or updating an existing care plan, our goal remains the same - ensure continuity of care and ensure positive outcomes.
Topics to discuss when developing a personal care plan:
Current eating, drinking habits and dietary concerns
Possible impairments with eyesight, hearing, and speaking
Various personal hygiene requirements and expectations
Daily sleeping and resting patterns and habits
Medication reminders and possible wound care
Mobility concerns and possible exercise/physical activity routines
Potential travel arrangements, locations, and frequencies
Household concerns regarding cleaning, laundry, and maintenance
Identify key points of contact - family, friends, doctors, or social workers
Review emergency plan in the event of unforeseen events or accidents
